Safety Management System
Hazard & risk controls, site-specific safety plans, inspection records, hold points and ISO 45001 alignment.
Introduction & Scope
This Safety Management System (SMS) defines the requirements and processes by which Jordan Rail Pty Ltd and its subcontractors manage health, safety and rail-safety risk across all operations on the rail network and at civil work sites.
It applies to all Jordan Rail operations - wet and dry hire, construction and maintenance, fabrication, transport and logistics, and mobilisation/demobilisation of plant - and to all employees, contractors and visitors at sites, yards and workshops under Jordan Rail's control.
The SMS operates alongside, and is supported by, the Jordan Rail Operating System (this platform), which serves as the system of record for all safety documents, inspections, hazard registers, incident reports and corrective actions. Health, safety and environmental requirements are governed by JRSE-110 Safety and Environmental Requirements, referenced throughout this document.
Safety & Health Policy
Jordan Rail is committed to providing a safe and healthy workplace, preventing work-related injury and ill health, and eliminating hazards and reducing OH&S risks. Safety objectives are established, monitored and reviewed at management level, and the safety policy is communicated to all workers and contractors.
- Provide and maintain safe plant, equipment and systems of work.
- Eliminate hazards and reduce OH&S risks through the hierarchy of controls.
- Consult and communicate with workers on matters affecting their health and safety.
- Ensure competence through induction, training, VOC and rail-safety competency management.
- Comply with all applicable WHS, rail-safety and regulatory requirements.
- Investigate incidents and near misses and implement corrective actions to prevent recurrence.
- Continually improve the SMS through measurement, audit and management review.
Company Operations & Safety Context
Jordan Rail provides heavy mobile plant and equipment for rail and civil construction. Operations occur in high-risk settings - active rail corridors, public roads, civil sites and workshops - where hazards include mobile plant interaction, working at heights, electrical, hot works, hazardous substances, rail access and fatigue.
Safety context is captured per project within the Operating System (clients, projects, rail networks, rail gauge and site locations) so that site-specific hazards and controls can be planned and recorded before work commences.
- Wet hire and dry hire of plant - mobile plant interaction, operator competency, pre-start checks.
- Construction and maintenance - working at heights, hot works, noise, vibration, hazardous substances.
- Transport and logistics - heavy vehicle operation, load restraint, fatigue management.
- Workshop and yard - machinery, lifting equipment, electrical test & tag, hazardous substances.
- Rail corridor works - rail access, train/track safety awareness, protection of personnel and the public.
SMS Framework (ISO 45001:2018)
The SMS is structured around the ISO 45001:2018 occupational health and safety management system principles and clause framework. The mapping below shows how each clause is satisfied by the Operating System.
- Clause 4 - Context: OH&S context and stakeholder needs captured per project; worker consultation.
- Clause 5 - Leadership: Management accountability via the Manager Overview; safety policy and objectives set and reviewed.
- Clause 6 - Planning: Hazard identification, risk assessment (SWMS, task risk assessments), OH&S objectives.
- Clause 7 - Support: Competency (RIW, VOC, inductions), communication, document control, infrastructure.
- Clause 8 - Operation: Operational controls via SWMS, pre-mob inspections, lockout/tagout, work procedures.
- Clause 9 - Performance evaluation: Inspections, hazard dashboard, incident analytics, audits, management review.
- Clause 10 - Improvement: Incident investigation, corrective actions, recurrence prevention.
Hazard Identification, Risk Assessment & Controls
Hazards are identified and risks assessed for every activity before work commences. Controls are selected using the hierarchy of controls (eliminate, substitute, engineer, administer, PPE). The Operating System maintains SWMS, a hazard dashboard, and plant/equipment risk assessments.
- Safe Work Method Statements (SWMS) - hazard register, controls, PPE, legislative references and version history.
- Hazard Dashboard - consolidates safety risks across registers and inspections.
- Task Risk Assessments - assessed per task with the hierarchy of controls applied.
- Plant and Equipment Risk Assessments - per unit before first deployment.
- Rail Safety Worker Competency - RIW roles and rail-safety training verified before rail access.
- PPE minimum requirements defined and enforced (high-visibility, hearing, eye, respiratory, gloves).
Preparing Site-Specific Safety Plans
For each project or major work scope, a Site-Specific Safety Plan (SSSP) is prepared before mobilisation. The SSSP translates this SMS into the specific hazards, controls and hold points for the site.
- Identify the site location, rail network/client safety conditions and access requirements.
- Identify and rate hazards and risks for the planned activities (SWMS and task risk assessments).
- Define controls using the hierarchy of controls for each identified hazard.
- Confirm competency requirements (RIW roles, VOC, tickets, medicals) for allocated personnel.
- Reference the SWMS, safety procedures, PPE and emergency response from JRSE-110.
- Establish plant pre-start, pre-mob inspection and hold-point requirements.
- Define incident, near-miss and hazard reporting and the responsible person for each.
- Review and approve the SSSP before mobilisation; communicate to the workforce via pre-start briefings.
Records of Inspection, Testing & Safety Activities
The Operating System maintains structured, referenced records of all safety inspection, testing and QA/QC activities. Each record carries a unique reference number, author, timestamp and supporting photos, and is retained as a permanent auditable record.
- Pre-Mobilisation Inspections (JR-PS) - 9-section checklist, defect register, supervisor sign-off.
- Battery Life Checks - start/battery/isolation status with auto-breakdown handling.
- Lifting Equipment register - assessor-led inspections and due-date tracking.
- Fire Extinguisher, First Aid, Spill Kit and Fall Protection registers - 3-monthly assessor, pre-mob and daily.
- Height & Slew Key register and EWP compliance records.
- Electrical Test & Tag and MSDS registers with currency status.
- SWMS - hazard register, controls and PPE with version history.
- Workshop & Site Pre-Start meetings with attendee sign-off and rich-text minutes.
- Tag-out / Lock-out records with photo evidence and return-to-service sign-off.
- Incident Reports with analytics, corrective actions and bulletin generation.
Hold Points & Witness Points
A Hold Point is a stage in the work sequence beyond which work must not proceed until the required safety inspection or verification has been completed and signed off by the nominated responsible person.
A Witness Point is a stage where the client, principal or safety officer is notified and given the opportunity to attend and witness the activity. Work may proceed if the witness does not attend after notification, unless the SSSP specifies otherwise.
- Pre-Mobilisation Inspection - supervisor sign-off is a hold point before plant leaves the yard.
- Battery Life Check - hold point before plant is released to site (start/battery/isolated checks).
- Lifting Equipment inspection - hold point before first lift after an assessor inspection.
- Tag-out removal - return-to-service is a hold point requiring authorised sign-off and photo evidence.
- Electrical Test & Tag - hold point before electrical equipment is placed in service.
- Hot works - permit/hold point before hot work commences and during fire watch.
- Rail access - protection and track-safety awareness hold point before entering the rail corridor.
- Client/safety officer witness points are nominated in the SSSP and notified through the system.
Incident Reporting, Investigation & Corrective Action
All incidents, injuries and near misses are recorded, investigated and actioned. Corrective actions are assigned, tracked to closure and analysed for recurrence. Serious incidents are reported to the relevant regulator (WHS and/or ONRSR) as required.
- Incident Reporting with analytics, corrective actions and safety bulletin generation.
- Incident Management Procedure & Manual (JRSE-110) — the detailed reporting, investigation, corrective action and follow-up procedure (Health & Safety → Incident Management → Procedure & Manual).
- Defect Reporting captured in any inspection flows into the repair/breakdown lifecycle when required.
- Incident site preservation and notification procedures per JRSE-110.
- Hazard Dashboard surfaces open risks across registers and inspections.
- Corrective actions tracked to closure with responsible person and verification — consolidated in the Action Tracking Register.
- Recurring-repair and incident-trend analysis feeds management review and improvement.
